OSHA Recordkeeping - Reporting Fatalities and MultipleHospitalizations
According to 29 CFR 1904.39, all employers covered under OSH Act must orally report to Occupational Safety and Health Administration (OSHA) within eight hours of:
- The death of an employee due to a work related incident
- The in-patient hospitalization of three or more employees as a result of a work-related incident
An employer can orally report the above incidents to the area office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor that is nearest to the site of the incident by:
- Telephone or in person
- Using OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742)
In case you can’t talk to a person in the area office or if the office is closed, you must report the fatality or multiple hospitalization incidents using the 800 number (given above).
Reporting Information Requirements:
An employer must provide the following information while reporting the fatality or multiple hospitalization incidents:
- The establishment name
- The location of the incident
- The time of the incident
- The number of fatalities and hospitalized employees
- The names of injured employees
- The contact person name with phone numbers
- A brief description of the incident
