Hierarchical Condition Categories (HCC)


Hierarchical Condition Categories (HCC) is defined as a risk adjustment model that employs a risk adjustment score to predict future healthcare costs for plan enrollees. Since 2004, the HCC model is used to determine reimbursement for various medical plans.

There are two types of HCC Models namely CMS-HCC and HHS-HCC.
