Course Description:

This interactive one and a half day course lead by FDA’s recently retired CDRH Recall Branch Chief Rita Hoffman who has more than 36 years’ experience in FDA compliance issues will provide the participants tools to minimize risk of regulatory enforcement actions.

During the seminar, she will explain proper handling of complaints reportable or non-reportable, product complaint handling and documentation, how and when to file Medical Device Reports (MDR), effective and appropriate communication with the appropriate regulatory agencies in the event of a recall. She will also discuss how to conduct a correction and removal actions to avoid a recall crisis, including required recordkeeping, expectation from FDA and other regulatory agencies in the event of a recall and key factors in implementing and maintaining compliance with the regulations and real life experiences of FDA.

Medical Device Reporting (MDR) and recall compliance are critical to the continue survival of all device manufacturers. The FDA is continuing their efforts to issue numerous FDA Warning Letters and serious enforcement actions, including criminal & civil penalties levied on companies that failed to properly report events and take proper corrective and removal actions. The number of device companies having their recall classified as a Class 1 (most severe) recall has surged in the past three years. Additionally, product liability and financial risks are staggering when companies fail to properly report and take action when required.

Seminar instructor Ms. Rita Hoffman is the former FDA CDRH Recall Branch Chief and has more than 36 years of FDA experience across the device, drug and veterinary industries.

This course will provide an understanding of MDR & recall compliance and the interrelationship of Complaint Handling, CAPA, and Risk Management processes. It will provide information on how you distinguish among a product complaint, a satisfaction complaint and a customer inquiry and does the FDA differentiate an instrument repair from a complaint?

Rita Hoffman, RAC. Managing Partner Regs & Recall Strategies, LLC .Ms. Hoffman has more than 36 years of FDA experience across the device, drug and veterinary industries. She has an intimate understanding of FDA regulatory and compliance issues from the perspective of both FDA and regulated industry. As an FDA compliance consultant, she provides clients with regulatory insight, advises on critical compliance deficiencies, performs compliance and new product audits, provides insight and guidance on recall strategies to the medical device industry, and advises on jurisdiction determinations for combination products.

Ms. Hoffman retired from the FDA in January 2011 as the Recall Branch Chief for the Center for Devices and Radiological Health (CDRH), where she was responsible for oversight and review for all medical device recalls. Ms. Hoffman held several positions including the Center for Drug Evaluation and Research (CDER) Jurisdiction Review Officer (providing guidance on drug/device product designation, combination products and co-packaging), Acting Associate Ombudsman, Small Business Liaison, and was a Policy Analyst for eight years in the Office of the Commissioner. She served as co-chair of RAPS’ Baltimore/Washington Metropolitan Area Chapter for 2-terms, and in 2008 was presented with the Special Recognition Award by RAPS.

Local Attractions

The museums are situated in Brussels’ downtown area on the Coudenberg. There are four museums connected with the Royal Museum, and two of them (the Museum of Ancient Art and the Museum of Modern Art, Brussels), are in the main building. The other two (the Constantin Meunier Museum and the Antoine Wiertz Museum) are dedicated to specific Belgian artists, are much smaller, and are located a few kilometers from the city center. The Royal Museum contains over 20,000 drawings, sculptures, and paintings, which date from the early 15th century to the present. The museum has an extensive collection of Flemish painting, among them paintings by Bruegel and Rogier van der Weyden, Robert Campin (the Master of Flémalle), Anthony van Dyck, and Jacob Jordaens. The museum is also proud of its "Rubens Room", which houses more than 20 paintings by the artist. The painting Landscape with the Fall of Icarus, long-attributed to Brueghel is housed here. The museum also hosts constant changing exhibitions.




Part of the Royal Museums for Art and History, the Musical Instrument Museum is internationally renowned for its collection of over 8,000 instruments. The museum's collection represents Belgian musical history (including Brussels' importance in the making of recorders in the 18th and 19th centuries and as the home of instrument inventor Adolphe Sax in the 19th century), European musical traditions, and non-European instruments. Mechanical instruments are shown in the basement, traditional instruments on the ground floor, the development of the modern orchestral instruments on the first floor, and keyboard and stringed instruments on the second floor. Visitors are given infrared headphones in order to listen to almost 200 musical extracts of the instruments on display. Among the notable pieces of the collection are the famous Rottenburgh Alto recorder, instruments invented by Adolphe Sax, a unique set of giant Chinese stone chimes, and the only existing copy of the luthéal, an instrument used by Ravel.




The Cathedral of St. Michael and St. Gudula is a Roman Catholic church on the Treurenberg Hill in Brussels, Belgium. The cathedral’s western façade with its three portals surmounted by gables and two towers are typical of the French Gothic style, but without rose window, which was replaced by a large window in the Brabantian Gothic style. The two towers, the upper parts of which are arranged in terraces, are attributed to the Flemish architect Jan Van Ruysbroeck (1470-1485), who also designed the tower of the Town Hall of Brussels. The south tower contains a 49-bell carillon by the Royal Eijsbouts bell foundry on which Sunday concerts are often given.




Mini-Europe is a miniature park located in Bruparck at the foot of the Atomium in Brussels, Belgium. Mini-Europe has reproductions of monuments in the European Union on show, at a scale of 1:25. Roughly 80 cities and 350 buildings are represented. The models cost as much as €350,000 to make (the Brussels Grand-Place). The park contains live action models (train, mills, eruption of Mount Vesuvius, Airbus, cable cars, etc.). A guide gives the details on all the monuments. At the end of the visit, the “Spirit of Europe” exhibition gives an interactive overview of the European Union in the form of multimedia games.




The Royal Museum of the Armed Forces and Military History occupies the two northernmost halls of the historic complex in Cinquantenaire Park in Brussels, Belgium. The collection originally consisted of a set of exhibits collected by an officer, Louis Leconte. Approximately 900 pieces were brought together, the collection was later heavily enriched by legacies, gifts and exchanges. Leconte was particular in choosing from equipment abandoned by the Germans in 1918. Today, it is possible to find uniforms, weapons, vehicles and military equipment of a number of periods and countries there.



The Atomium is a building in Brussels originally constructed for Expo '58, the 1958 Brussels World's Fair. Designed by the engineer André Waterkeyn and architects André and Jean Polak, it stands 102 m tall. Its nine 18 m (59 ft) diameter stainless steel clad spheres are connected so that the whole forms the shape of a unit cell of an iron crystal magnified 165 billion times. Tubes connect the spheres along the 12 edges of the cube and all eight vertices to the center. They enclose escalators and a lift to allow access to the five habitable spheres which contain exhibit halls and other public spaces. The top sphere provides a panoramic view of Brussels. CNN named it Europe's most bizarre building.




Bois de la Cambre is an urban public park on the edge of the Sonian Forest in Brussels, Belgium. It has an area of 1.23 square kilometers. The park lies to the south of the Brussels-Capital Region, and in the municipality of the City of Brussels. It is linked to the rest of the municipality by Avenue Louise, which was built at the same time the park was laid out in 1861. It comprises a small lake with an island in the center called Robinson's island.
